Job summary

Creekstone Health and Wellness in Kissimmee, FL is seeking a compassionate Speech-Language Pathologist (SLP) to evaluate and treat residents in a skilled nursing setting.

You will develop individualized plans, document progress, and collaborate with physicians, nurses, and therapists to maximize patient outcomes and quality of life.

Qualifications

• Master’s degree in Speech-Language Pathology from an accredited program.
• Current state licensure as a Speech-Language Pathologist.
• CCC from ASHA preferred.
• Experience in rehabilitation or subacute care is a plus.
• Strong communication and interpersonal skills.
• Ability to work collaboratively in a team environment.

Responsibilities

• Conduct thorough evaluations of patients’ speech, language, cognitive-communication, and swallowing abilities.
• Develop individualized treatment plans based on assessment results and patient needs.
• Implement and document therapeutic interventions to support patient progress and rehabilitation goals.
• Collaborate with a multidisciplinary team to provide holistic care.
• Educate patients and families about communication disorders and rehabilitation techniques.
• Maintain accurate documentation of patient progress per regulatory standards.
• Participate in staff meetings and contribute to team-based care discussions.
• Stay current with best practices in speech-language pathology.

• Current state licensure as a Speech-Language Pathologist.

• Certificate of Clinical Competence (CCC) from the American Speech-Language-Hearing Association (ASHA) preferred.

• Previous experience in a rehabilitation or subacute care setting is a plus.
