Job summary

HealthPro Heritage is seeking a Speech Language Pathologist to fill a PRN role at our senior living facility in Thornwood, NY. This position focuses on accurate assessments, individualized therapy plans, and family education, with a schedule that supports M-F hours and no evenings or weekends.

Join a therapist-led organization delivering compassionate care across multiple settings including nursing facilities, hospitals, and home care. ASHA-certified candidates preferred; licensure required.

Qualifications

• Education: Degree in Speech Language Pathology from an accredited program.
• Licensure: Valid state licensure as a Speech Language Pathologist, or license eligible.
• Certification: Current state licensure as a Speech-Language Pathologist. ASHA CCC-SLP certification.
• Skills: Strong assessment, diagnostic, and therapeutic abilities; good communication and teamwork; proficient with diagnostic tools and EHRs.

Responsibilities

• Patient assessment: conduct evaluations of speech, language, voice and swallowing, with standardized tests and observations.
• Treatment and education: implement evidence-based therapies, monitor and adjust plans, educate patients and families.
• Documentation: maintain accurate records of evaluations, progress, and plans in EHRs.
• Collaboration: work with healthcare providers and participate in multidisciplinary team meetings.

• Collaboration: Work with healthcare providers to ensure effective patient care and participate in multidisciplinary team meetings as needed.

Qualifications

• Education: Degree in Speech Language Pathology from an accredited institution.
• Licensure: Valid state licensure as a Speech Language Pathologist, or license eligible
• Certification: Current state licensure as a Speech-Language Pathologist. Certification from the American Speech-Language-Hearing Association (ASHA) (CCC-SLP).
• Skills: Strong assessment, diagnostic, and therapeutic abilities with excellent communication and interpersonal skills. Proficient in using diagnostic tools, therapy equipment, relevant software, and electronic health record systems, with a proven ability to work collaboratively in a multidisciplinary team.
