Job summary

PASCO/SW Home Health & Hospice in Colorado is seeking a Speech-Language Pathologist to provide patient-centered therapy in homes and communities. You will evaluate, plan, and deliver services, collaborating with families and the care team to improve communication, safety, and quality of life.

You will supervise aides when needed, maintain documentation, and participate in team activities. A Colorado license, CPR, driver’s license, and 1 year of SLP experience are required.

Qualifications

  • Master’s or doctoral degree in Speech-Language Pathology.
  • Current licensure to practice in the State of Colorado as a Speech-Language Pathologist.
  • Minimum 1 year of clinical experience in Speech-Language Pathology.
  • Strong verbal, written communication and organizational skills.
  • Current CPR certification and a valid driver’s license with reliable transportation.

Responsibilities

  • Conduct comprehensive speech-language evaluations and assess home environments as appropriate.
  • Develop and update individualized plans of care with patients, caregivers, and the care team.
  • Provide speech-language therapy services as ordered by the physician.
  • Identify deficits and implement targeted treatment plans.
  • Utilize specialized techniques and equipment to improve outcomes.
  • Educate and counsel patients, families, and caregivers.
  • Maintain accurate clinical documentation and communicate with the care team.
  • Participate in case conferences, in-services, and quality improvement initiatives.
  • Identify needs for additional services and make referrals as appropriate.
  • Supervise home health aides in tasks related to the therapy plan when applicable.
