Job summary

Hudson Wide Health Care in New York is seeking a dedicated Speech-Language Pathologist (SLP) to join our Skilled Nursing Facility team. You will assess, diagnose, and treat speech, language, and swallowing disorders, delivering patient-centered care.

Collaborate with residents, families, and interdisciplinary teams to develop individualized treatment plans and monitor progress, ensuring high-quality outcomes and compassionate service.

Qualifications

• CCC-SLP certification from ASHA required.
• Experience in SNF settings beneficial.
• Strong communication and interpersonal skills.
• Compassionate and solution-oriented approach to care.

Responsibilities

• Provide therapy services as prescribed by the referring physician.
• Perform comprehensive patient assessments and develop individualized treatment plans.
• Conduct speech/language screenings and swallowing evaluations as needed.
• Monitor patient progress and adjust treatment plans accordingly.
• Communicate effectively with families and healthcare team members regarding care and progress.
• Ensure appropriate follow-up on diagnostic findings with multidisciplinary coordination.

Position Overview:

As a Speech-Language Pathologist, you will play a vital role in enhancing the quality of life for our residents by assessing, diagnosing, and treating speech, language, and swallowing disorders. You’ll collaborate closely with patients, families, and interdisciplinary team members to deliver exceptional, patient-centered care.

Key Responsibilities:
