Qualifications

• Active, unencumbered license from the state of practice is required.
• Ability to educate patients on maintaining achieved levels of function.

Responsibilities

• Facilitate achievement of patient, physician, and therapy goals in a SNF setting.
• Instruct patients in maintaining achieved level of function after discharge.

Skills
