Houston Transitional Care—Houstonidence Opco, LLC seeks a Staff Speech-Language Pathologist to evaluate and treat residents, communicate with families and health professionals, and maintain compliant documentation.

The role includes supervising CFY Clinical Fellows or students, developing individualized treatment plans, and participating in interdisciplinary teams to support discharge planning and quality outcomes.

Qualifications

• Bachelor’s degree in Speech-Language Pathology; Master’s/Doctorate preferred.
• Licensed Speech-Language Pathologist in the state; license in good standing.
• Proficiency with Net Health and Point Click Care.
• Ability to read and implement policy, procedures, and medical orders.

Responsibilities

• Evaluate residents to determine speech therapy needs and rehab potential.
• Develop, implement, and update individualized treatment plans.
• Supervise CFY Speech Clinical Fellows and students.
• Communicate progress and discharge planning with physicians, families, and staff.
• Complete timely, accurate documentation per requirements.
• Educate residents, families, and caregivers on goals and home programs.
• Recommend durable medical equipment when needed.
• Participate in care conferences and quality initiatives.
