Job summary

Pinnacle Home Care, Florida’s largest Medicare-certified home health provider, is seeking a PRN Speech Language Pathologist (SLP) to join our award-winning team and deliver compassionate in-home care.

Responsibilities include evaluating speech and language abilities, conducting assessments including OASIS, developing treatment plans, collaborating with physicians and the interdisciplinary team, educating clients and families, and documenting progress.

Qualifications

• Graduate of an accredited Speech Therapy program.
• Licensed Speech Therapist in Florida.
• Minimum of two years of clinical experience as a Speech Therapist.
• Current BLS Certification.
• Valid driver’s license and insured, reliable vehicle.

Responsibilities

• Evaluate clients’ speech, language, and hearing abilities; recommend strategies to enhance communication and hearing.
• Conduct initial and ongoing comprehensive assessments, including OASIS evaluations.
• Develop and revise treatment plans in consultation with physicians as necessary.
• Collaborate with interdisciplinary team members on care plans and provide speech pathology training to team members as needed.
• Educate clients and families on speech therapy techniques and related challenges.
• Document all evaluation data, treatments, and client progress promptly.
• Supervise Home Health Aides every 14 days when Speech Therapy is the sole discipline involved.
• Communicate regularly with physicians, ensuring orders are appropriate and addressing any changes in client condition.
• Participate in the periodic revision of care plans, updating the physician on client status and obtaining additional orders as required.
