Pinnacle Home Care in Florida is hiring a Speech Therapist (ST/SLP) PRN to join our compassionate team. You will provide personalized care to clients in Winter Haven and surrounding areas, evaluating their speech and language abilities and developing tailored treatment plans.

Qualifications

Pinnacle Home Care, Florida’s largest Medicare-certified home health provider, has been delivering high-quality, patient-centered care for over two decades, and we’re looking for PRN Speech Language Pathologist (SLP) to join our award-winning team.

Responsibilities

• Evaluate clients’ speech, language, and hearing abilities, recommending strategies to enhance communication and hearing.
• Conduct initial and ongoing comprehensive assessments, including Outcome and Assessment Information Set (OASIS) evaluations.
• Develop and revise treatment plans in consultation with physicians as necessary.
• Collaborate with interdisciplinary team members on care plans and provide speech pathology training to team members as needed.
• Educate clients and families on speech therapy techniques and related challenges.
• Document all evaluation data, treatments, and client progress promptly.
• Supervise Home Health Aides every 14 days when Speech Therapy is the sole discipline involved.
• Communicate regularly with physicians, ensuring orders are appropriate and addressing any changes in client condition.
• Participate in the periodic revision of care plans, updating the physician on client status and obtaining additional orders as required.

Qualifications

• Graduate of an accredited Speech Therapy program.
• Licensed Speech Therapist in Florida.
• Minimum of two years of clinical experience as a Speech Therapist.
• Current BLS Certification.
• Valid driver’s license and insured, reliable vehicle.
• Strong emotional resilience and physical stamina, including the ability to stand, stoop, and bend frequently.
• Excellent judgment, communication, and observational skills.
• Self-motivated with the ability to work independently.
