Job summary

Slaton Care Center in Slaton, TX is seeking a Per Diem Speech Language Pathologist to join our in-house rehab team. The SLP will evaluate swallowing and cognitive deficits, develop individualized treatment plans, and provide AAC and education to residents and families.

Active Texas licensure and ASHA certification are required. Per diem schedule offers flexibility and opportunities to collaborate with nursing and restorative staff, with a focus on person-centered care and positive outcomes.

Qualifications

• Active Texas license in Speech-Language Pathology.
• Current ASHA Certification.
• Good standing with regulatory agencies.

Responsibilities

• Evaluate residents for swallowing/cognitive deficits.
• Assist residents to achieve goals in cognition/swallowing.
• Educate residents and caregivers.
• Develop and implement individualized treatment plans.
• Provide AAC systems for severe language disorders.
• Train clients, families, caregivers, and professionals.
• Coordinate with physicians for diagnostic swallowing procedures.
• Demonstrate current speech therapy standards of practice.
• Communicate changes in residents’ condition.
• Maintain accurate billing records and documentation.
• Respond to family requests promptly.
• Coordinate training with Restorative Nursing and staff.
• Ensure no abuse or neglect of residents.
• Protect confidentiality of patient information.
• Comply with all laws and professional requirements.

We have an amazing opportunity for a Per Diem (PRN) Speech Language Pathologist!

We are looking for a SLP to join our in-house team of outstanding professionals that service our residents through person-centered care to achieve positive outcomes!

SLP Responsibilities

• Evaluate Residents conditions for swallowing or cognitive deficits.
• Help Residents in their goals to improve Cognition or Swallowing
• Education for Residents and Caregivers
• Provide skilled, medically necessary speech and language therapy assessments as well as develop and implement individualized treatment plans to increase client’s independence and functioning.
• Provide augmentative and alternative communication (AAC) systems for individuals with severe expressive and/or language comprehension disorders.
• Provide training and education to clients, family, caregivers, and other professionals as a subject matter expert.
• Coordinate and collaborate with other professionals for diagnostic swallowing procedures (MBSS, FEES) as ordered by physician.
• Demonstrate competent technical skills according to current Speech Therapy standards of practice.
• Communicate changes in resident’s condition.
• Provide accurate and timely billing records and supporting clinical documentation.
• Respond to family requests and concerns timely.
• Coordinate and provide training with Restorative Nursing and other Care staff.
• Assure resident abuse or neglect does not occur.
• Observe confidentiality and safeguards for all patient information.
• Complies with federal, state, and local legal and professional requirements.

SLP Skills

• SNF/Long term care experience preferred.
• Current knowledge of treatment practices
• Knowledge of Net Health Electronic Documentation System preferred.
• Ability to manage patients with different types of personalities.

SLP Requirements

• Active/Valid Texas license in Speech-Language Pathology
• Current ASHA Certification
• In good standing with all regulatory agencies and licensing boards
