Job summary

Creative Solutions In HealthCare is seeking a Per Diem Speech Language Pathologist (SLP) to join our in-house team serving residents with person-centered care.

The role requires active Texas licensure and ASHA certification, with flexible schedules and competitive compensation. Ideal for new grads seeking in-house rehab exposure in a SNF/long-term care setting.

Qualifications

• Active/Valid Texas license in Speech-Language Pathology.
• Current ASHA Certification.
• In good standing with all regulatory agencies and licensing boards.

Responsibilities

• Evaluate residents for swallowing or cognitive deficits.
• Help residents in goals to improve cognition or swallowing.
• Educate residents and caregivers.
• Provide skilled speech-language therapy assessments and develop individualized treatment plans.
• Implement AAC systems for expressive and language disorders.
• Provide training to clients, family, caregivers, and other professionals.
• Coordinate with other professionals for diagnostic swallowing procedures (MBSS, FEES).
• Demonstrate competent clinical skills per standards of practice.
• Communicate changes in resident status.
• Maintain billing records and clinical documentation.
• Respond to family requests in a timely manner.
• Coordinate training with Restorative Nursing and care staff.
• Ensure no abuse or neglect of residents occurs.
• Observe confidentiality of patient information.
• Comply with regulatory requirements.

Job description

We have an amazing opportunity for a Per Diem (PRN) Speech Language Pathologist!

We are looking for a SLP to join our in-house team of outstanding professionals that service our residents through person-centered care to achieve positive outcomes!

SLP Responsibilities

• Evaluate Residents conditions for swallowing or cognitive deficits.
• Help Residents in their goals to improve Cognition or Swallowing
• Education for Residents and Caregivers
• Provide skilled, medically necessary speech and language therapy assessments as well as develop and implement individualized treatment plans to increase client's independence and functioning.
• Provide augmentative and alternative communication (AAC) systems for individuals with severe expressive and/or language comprehension disorders.
• Provide training and education to clients, family, caregivers, and other professionals as a subject matter expert.
• Coordinate and collaborate with other professionals for diagnostic swallowing procedures (MBSS, FEES) as ordered by physician.
• Demonstrate competent technical skills according to current Speech Therapy standards of practice.
• Communicate changes in resident’s condition.
• Provide accurate and timely billing records and supporting clinical documentation.
• Respond to family requests and concerns timely.
• Coordinate and provide training with Restorative Nursing and other Care staff.
• Assure resident abuse or neglect does not occur.
• Observe confidentiality and safeguards for all patient information.
• Complies with federal, state, and local legal and professional requirements.

SLP Skills

• SNF/Long term care experience preferred.
• Current knowledge of treatment practices
• Knowledge of Net Health Electronic Documentation System preferred.
• Ability to manage patients with different types of personalities.

SLP Requirements

• Active/Valid Texas license in Speech-Language Pathology
• Current ASHA Certification
• In good standing with all regulatory agencies and licensing boards
