Job summary

CenterWell Home Health in Connecticut is seeking a Speech Language Pathologist to deliver in-home therapy. You will evaluate and treat communication and swallowing disorders, and collaborate with a care team to restore independence and improve quality of life.

You will develop and review treatment plans, monitor progress, and ensure documentation meets policy standards, enabling patients to regain function in their daily activities.

Qualifications

• CCC-SLP certification per ASHA requirements.
• Minimum six months of speech-language pathology experience.
• Current, unrestricted SLP license and CPR certification.
• Valid driver's license and reliable transportation required.

Responsibilities

• Evaluate, direct and provide speech-language pathology services in home or facility.
• Develop and review the Plan of Treatment and Plan of Care.
• Assess and treat disorders of speech, language, voice, hearing, and swallowing.
• Administer and interpret diagnostic tests including audiologic screening.
• Record changes in patient condition and report to supervisor/physician.
• Instruct patients on alternative communication methods when appropriate.
• Provide counsel to patients, families and healthcare staff.
• Submit documentation: evaluations, visits, care plans, progress reports.
• Participate in care coordination and discharge planning.
• Maintain confidentiality and proper information sharing per policy.

As a Home Health Speech Language Pathologist, You Will

• Evaluate, direct and provide speech/language pathology service to patients in the home or facility
• Participate in the development and periodic review of the Plan of Treatment and Plan of Care.
• Use professional skills and judgment in assessing and treating disorders of speech, voice, language, hearing and swallowing to prevent, identify, evaluate and minimize the effects of such disorders and conditions.
• Administer and interpret diagnostic tests and applications of therapeutic treatments including audio logic screening.
• Observe, record and report changes in the patient's condition and response to treatment to supervisor and/or the physician.
• Provide instruction and training to patients in use of alternative communication systems when appropriate.
• Provide counsel and instruction to patients, families and healthcare staff.
• Maintain and submit documentation as required by the Company and/or facility. Prepare and submit written reports of evaluations, visits, summaries, care plans, care coordination activities, and progress reports as required by Company policy.
• Participate in care coordination activities and discharge planning.
• Maintain the highest standards of professional conduct concerning information that is confidential in nature. Share information only when the recipient's right to access is clearly established and the sharing of such information is clearly in the best interests of the patient.

Required Qualifications

• Meet the education and experience requirements for Certification of Clinical Competence in Speech Language Pathology or Audiology granted by ASHA
• Minimum six months experience as a speech therapist / speech language pathologist
• Current and unrestricted license
• Current CPR certification
• A valid driver's license, auto insurance, and reliable transportation are required.

Pay Per Visit/Unit Rate

$60.00 - $83.00

TB Statement

This role is considered patient facing and is part of Humana's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.

Driving Statement

This role is part of Humana's driver safety program and therefore requires an individual to have a valid state driver's license and are expected to maintain personal vehicle liability insurance. Individual must carry vehicle insurance in accordance with their residing state minimum required limits, or $25,000 bodily injury per person/$25,000 bodily injury per event /$10,000 for property damage or whichever is higher.

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.
