Job summary

JTC Home Health Inc. is seeking a compassionate Speech-Language Pathologist to deliver in-home therapy on a per diem, fee-for-service basis in San Diego and surrounding counties.

The role offers flexible scheduling, autonomy, and competitive compensation with direct patient care in clients' homes. You will conduct comprehensive evaluations, develop and implement POC treatment plans, educate patients and caregivers, and coordinate with the interdisciplinary team, while meeting OASIS and EMR

Qualifications

• Master's degree in Speech-Language Pathology from an accredited program.
• California SLP license is current and unencumbered.
• CCC-SLP from ASHA is required.
• Minimum of 1 year of SLP experience; 6 months in a certified Home Health agency preferred.
• Knowledge of Medicare/Medicaid regulations; OASIS experience preferred.
• Must have reliable car, valid driver's license and auto insurance.
• Ability to travel throughout San Diego and surrounding counties.

Responsibilities

• Conduct comprehensive initial and ongoing speech-language pathology evaluations and assessments in the patient's home.
• Develop, implement, and document individualized treatment plans for speech, language, cognitive, and swallowing disorders.
• Provide patient/caregiver education and training on home programs and compensatory strategies.
• Coordinate with physicians, nurses, and other healthcare team members.
• Complete timely OASIS and documentation using an EMR system.
• Maintain open communication with the office team.

Compensation & Benefits

• Evaluation (Initial/ROC): $120.00 per visit
• Routine Visit: $110.00 per visit
• Mileage Reimbursement: $0.70 per mile (paid between patient visits)

Responsibilities

• Conduct comprehensive initial and ongoing speech-language pathology evaluations and assessments in the patient's home environment.
• Develop, implement, and document individualized treatment plans (POC) for patients with speech, language, cognitive, and swallowing disorders (dysphagia).
• Provide patient/caregiver education and training on home programs and compensatory strategies.
• Coordinate with physicians, nurses, and other members of the interdisciplinary healthcare team.
• Complete timely and accurate OASIS and documentation requirements using an Electronic Medical Record (EMR) system.
• Maintain open and professional communication with the JTC Home Health office team.

Qualifications

• Master's degree in Speech-Language Pathology from an accredited program.
• Current and unencumbered California SLP license.
• Certificate of Clinical Competence (CCC-SLP) from ASHA required.
• Minimum of one (1) year of experience as an SLP, with at least six (6) months in a certified Home Health agency preferred.
• Knowledge of Medicare/Medicaid regulations and documentation requirements (OASIS experience preferred).
• Must have a reliable car, valid driver's license, and current auto insurance.
• Ability to travel throughout San Diego and surrounding counties.
