Sans Souci Rehabilitation & Nursing Center is seeking a Speech Language Pathologist (SLP) to provide and direct speech therapy services, including assessments, program development, and in-service education under physician orders. Responsibilities

  • Provides Speech Therapy services to patients and initiates referrals when appropriate.
  • Conducts screenings of patients at regular intervals to determine need for intervention/treatment.
  • Evaluates patients to obtain data necessary for treatment planning and implementation; conducts specialized evaluations as indicated.
  • Interprets and communicates evaluation findings to patients, family, and other team members.
  • Establishes individual patient schedules in conjunction with other team members.
  • Develops treatment plans by identifying a problem list with short and long-term goals and methods to achieve identified goals.
  • Coordinates treatment plan with patient, family, and other team members.
  • Monitors patient response to intervention and modifies treatment as indicated to attain goals.
  • Recommends and obtains appropriate equipment to improve patient communication; instructs patient, family and staff in use of augmentative communication devices.
  • Participates in discharge planning.
  • Develops home or community programming to maintain and enhance patient performance in their environment.
  • Documents results of assessment, treatment, follow-up and termination of services.
  • Supervises students and volunteers in accordance with state regulations.
  • Maintains continuing education and professional development.
  • Records indirect and billable treatment times and submits weekly/monthly statistics to regional office.
  • Provides treatment in accordance with standards of practice and department procedures.
  • Complies with Infection Control, Universal Precautions and OSHA standards.
  • Assists with Quality Assurance programs as required.
  • Adheres to confidentiality standards.
  • Performs other duties as assigned.
