ROLE OVERVIEW And PURPOSE

The Speech Language Pathologist works to support the mission and vision of Neighborhood Healthcare by providing speech language pathology services for Participants. This role will design and develop Participant care plans based on speech language pathology assessments. Clinical supervision for this role is provided within the manager’s licensed scope of practice, while designated discipline-specific leads or clinical coordinators provide clinical oversight for services outside the manager’s licensed discipline.

This role will design and develop Participant care plans based on speech language pathology assessments. Clinical supervision for this role is provided within the manager’s licensed scope of practice, while designated discipline-specific leads or clinical coordinators provide clinical oversight for services outside the manager’s licensed discipline.

Responsibilities

  • Screens and evaluates Participants for speech, language, cognitive-communication, voice, swallowing, and hearing needs according to the American Speech Language Hearing Association guidelines.
  • Develops and implements individualized plans of care to improve or restore communication, cognitive-linguistic function, and swallowing abilities; promotes independence; and support Participants in achieving their highest functional level.
  • Evaluates the effectiveness of treatment plans and modifies goals, as needed
  • Communicate with staff regarding Participants’ needs in areas of communication strategies, dietary texture modifications, safe swallowing practices, and cognitive support, as needed.
  • Communicate with family members and caregivers to ensure that therapeutic recommendations—such as communication aids, diet modifications, and assistive technology—are clearly understood, feasible, and can be appropriately supported at home.
  • Makes recommendations and instructs Participants and caregivers on the use and maintenance of augmentative and alternative communication (AAC) devices and compensatory strategies to promote communication and safety.
  • Identifies malfunctioning AAC or adaptive equipment and takes appropriate action to repair or replace items.
  • Ensuring that device trials, education, and loan documentation are accurately recorded per department policy.
  • Collaborate, as indicated, with skilled nursing facility and assisted living staff regarding the communication, cognitive, and swallowing needs of Participants.
  • Provides speech language therapy targeting areas such as but not limited to expressive and receptive language, articulation, cognitive-linguistic skills, dysphagia management, and voice therapy.
  • Provides written home programs, visual support, and caregiver education to facilitate carryover of therapy goals outside of sessions.
  • Makes home and or community visits to perform evaluations and conduct educational sessions as needed and indicated in the plan of care; documents activities and Participant/caregiver responses in the Participant’s chart.
  • Delegates components of therapy or supportive tasks to SLP Assistants (if applicable), consistent with the California Speech-Language Pathology Practice Act and ASHA supervision standards.
  • Discusses plan of care with the multi-disciplinary team as needed to integrate services with overall plan of care for Participants.
  • Provides guidance to staff and team members that is consistent with state laws and company policies
  • Documents all procedures performed in the Participant’s medical chart according to NHC policies and procedures; including but not limited to: Assessment, care plan, daily encounter notes, education, discharge summary.
  • Keeps abreast of all regulations and standards to ensure compliance with orders or directives issued by duly constituted government and regulatory agencies (PACE, CMS, DHCS)
  • Participates in training programs for staff and caregivers
  • Enhances professional growth and development through participation in educational programs, current literature, in-service meetings, and workshops

Education/Experience

  • Graduate of a Master of Speech Language Pathology program, accredited by the American Speech Language Hearing Association, required
  • Valid CA Speech Language Pathology License, required
  • Valid BLS certification from approved American Health Association provider required upon hire
  • One year experience of providing and coordinating speech language pathology services for a frail or elderly population, required

ADDITIONAL QUALIFICATIONS (Knowledge, Skills And Abilities)

  • Excellent verbal and written communication skills, including superior composition, typing and proofreading skills
  • Ability to interpret a variety of instructions in written, oral, diagram, or schedule form
  • Knowledgeable about and experience with principles and methods for treatment and rehabilitation of functional impairments
  • Ability to establish and maintain therapeutic relationships and to participate in a multi-cultural, multidisciplinary team
  • Ability to successfully manage multiple tasks simultaneously
  • Excellent planning and organizational ability
  • Ability to work as part of a team as well as independently
  • Ability to work with highly confidential information in a professional and ethical manner

Physical Requirements

  • Ability to lift/carry 25 lbs./weight
  • Ability to stand for long periods of time

Pay grade 10: $45.91 - $67.93/hr. depending on experience, education and additional qualifications.

The posted salary range reflects the designated pay grade for this position. While this range represents the broader classification of the role, actual compensation will be based on several factors, including but not limited to: the candidate’s overall knowledge, skills, and experience, market data and industry benchmarks, internal equity within the organization, Budgetary considerations and organizational needs.

As a result, placement within the range is not guaranteed, and the full pay grade range may not be utilized.
