Key Responsibilities:

• Screen and evaluate patients with communication, cognitive, or swallowing disorders, developing appropriate care plans in accordance with regulations and clinical standards.
• Provide rehabilitative, skilled, and medically necessary treatments for patients with communication, cognitive, or swallowing disorders.
• Evaluate treatment effectiveness and modify services as needed; refer patients based on outcomes.
• Offer consultation and counseling to patients, families, caregivers, and other providers regarding speech disorders.
• Prepare comprehensive discharge summaries in compliance with regulatory and clinical standards.
• Accurately document all required information in electronic records, maintaining patient confidentiality and records in line with company policy.
