Collage Home and Community Rehabilitation Partners in Anaheim, CA is seeking experienced Speech Language Pathologists to provide patient-centered care in clients' homes and communities, with PRN positions available.

Therapists work with interdisciplinary teams to create integrated plans and address neurological and ortho-related communication and swallowing needs, delivering evidence-based therapy in patients' own homes.

Qualifications

• Master’s degree in Speech Pathology and CA state licensure required; ASHA CCC preferred.
• Experience treating neurologically related disorders strongly preferred.
• Excellent planning, organization, writing, and time management skills required.
• Ability to communicate with families, professionals, and co-workers.

Responsibilities

• Evaluation and treatment of impairments in speech, language, executive functioning, cognition, dysphagia, and augmentative communication.
• Provide patient-centered therapy in clients\' homes and communities.
• Collaborate with interdisciplinary teams to develop integrated treatment plans.
• Maintain clear documentation and communication with families and care teams.

We are looking for experienced Speech Language Pathologists who want to provide truly patient-centered care in Anaheim, CA surrounding areas - with PRN positions available that accept patients for the duration of their stay available. Our interdisciplinary teams create integrated treatment plans that are practiced in patients' homes and communities. Our therapists are creative in their approach and incorporate a variety of activities to ignite interest while addressing functional goals. This evidence-based approach results in durable outcomes.

Qualifications:

Completion of Master’s degree in Speech Pathology and state licensure is required along with a certificate of clinical competence from ASHA. Experience in the treatment of neurologically related disorders is strongly preferred. Excellent planning, organization, writing, and time management skills are required. Ability to communicate with families, professionals, and co-workers is a must.
