Job summary

Evolve Therapy Services, LLC is seeking a Speech Language Pathologist for an in-house Skilled Nursing Facility in Syracuse, NY. This position can be full-time, part-time, or PRN, offering diverse scheduling to meet professional and patient needs.

You will plan, organize, and direct speech-language pathology services, ensure compliance with CMS standards, and collaborate with administrators to ensure high-quality resident care. Licensing and ongoing CEUs are required.

Qualifications

• Master's degree in Speech Language Pathology is required.
• CCC certification is required and must be active.
• Active state licensure as SLP in practicing states.
• Ongoing CEU to maintain licensure and certification.

Responsibilities

• Provide quality speech therapy services and initiate referrals when appropriate.
• Evaluate and obtain data for treatment planning and implementation.
• Develop treatment plans with target lists and short-/long-term goals.
• Instructs patient, family, and staff in speech-language procedures for carryover.
• Complete compliant CMS documentation in NetHealth EMR systems.
• Oversee FEES/MBS protocols and coordinate site programs.
• Participate in resident screening to identify needs promptly and with clinical excellence.

Job Position:

Speech Language Pathologist for an In-house Skilled Nursing Facility

Job Type:

Full Time, Part Time, or PRN

Job Purpose:

To assist in planning, organizing, developing, and directing Speech Language Pathology services in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be direction by Administrator, to assure the highest degree of quality resident care is always maintained.

Job Functions:

• To provide quality speech therapy services and initiate referrals when appropriate, in accordance with established standards of practice, therapy department procedures and productivity standards.
• To evaluate and obtain data necessary for treatment planning and implementation
• To develop a treatment plan that includes a target list, short-term, and long-term goals
• Instructs patient, family, and staff in appropriate speech-language procedures to allow carry over of therapeutic remediation and goals in daily activities
• To complete compliant and medically necessary documentation from CMS expectations in NetHealth EMR systems
• Oversight and implementation of proper protocols for FEES/MBS with coordination of site programs
• Participants in screening protocols for establishing needs of residents are being met timely and with clinical excellence

Education & Qualifications:

• Master's degree in Speech Language Pathology
• Certificate of Clinical Competency
• MUST have active SLP license in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification
