BAYADA Home Health Care is seeking an experienced Speech Language Pathologist (SLP) for a per diem position.

This role involves performing home health visits for our Senior Living Home Health office across communities in Uniontown and surrounding areas.

Prior home care experience as an SLP is a plus but not required.

As a home care SLP, you will be part of a multidisciplinary team providing rehabilitative care and skilled nursing, helping clients receive necessary medical care to stay at home.

• Make home visits to clients in designated areas.
• Perform evaluations to assess, diagnose, and treat speech, language, communication, or swallowing issues.
• Develop treatment plans based on evaluations, physician’s orders, and goals.
• Assess and update care plans; collaborate with clinicians in the care team.
• Educate patients and families on rehabilitative activities to promote health and safety.
• Document observations and interventions accurately using electronic tablets.

Qualifications:

• Current Ohio SLP license.
• Certificate of Clinical Competence from ASHA or equivalent qualifications, with eligibility to take the ASHA exam.
• Solid computer skills; experience with EMR systems preferred.
