Life Care Home Health is hiring a Speech Language Pathologist (SLP) to evaluate and treat patients with speech, language, cognitive, hearing, and swallowing disorders in a home health setting. You will work with physicians, caregivers, and an interdisciplinary team to maximize independence and quality of life.

Responsibilities include developing plans of care, providing therapy, documenting patient progress, and maintaining compliance with policies and payer requirements.

Qualifications

• Master's or Doctorate degree in Speech-Language Pathology from an accredited college or university.
• Current Speech Language Pathologist license in good standing is required.
• Minimum of two (2) years of experience in a clinical setting; home health experience preferred.
• Valid driver's license and reliable transportation for home visits.

Position Summary

As a Speech Language Pathologist (SLP) , you will evaluate, develop, and implement individualized treatment plans for patients experiencing speech, language, cognitive, hearing, and swallowing disorders. You'll collaborate with physicians, caregivers, and the interdisciplinary team to help patients achieve their highest level of independence and quality of life.

Requirements

Experience

• Minimum of two (2) years of experience as a Speech Language Pathologist in a clinical setting
• Home health experience preferred

Education

• Master's or Doctorate degree in Speech-Language Pathology from an accredited college or university

Licensure & Certifications

• Current Speech Language Pathologist license in good standing
• Current CPR certification
• Valid driver's license
• Reliable transportation with current auto liability insurance

• Work primarily in patients' homes under varying environmental conditions
• Ability to travel locally and maintain a flexible schedule
• May be exposed to bloodborne pathogens, bodily fluids, infectious diseases, and varying weather conditions
