Job description

BAYADA Home Health Care is currently seeking an experienced Speech Language Pathologist (SLP) for a part-time or PRN opportunity performing home health visits for our office. This office services adult and geriatric patients in senior living communities throughout Loudoun County, VA.

Prior home care experience as a Speech Language Pathologist is a plus but not required.

As a home care Speech Language Pathologist, you will be an integral member of a multi-disciplinary healthcare team that provides rehabilitative care and skilled nursing to clients, enabling them to receive necessary medical care while remaining at home.

Responsibilities include:

1. Making home visits to clients in designated geographic territories.
2. Performing evaluations to assess, diagnose, and treat speech, language, communication, or swallowing difficulties.
3. Developing treatment plans based on evaluations, physician’s orders, and goals.
4. Assessing and revising care plans, and collaborating with BAYADA clinicians in multidisciplinary care planning.
5. Educating patients and families in rehabilitative care and activities to promote health, safety, and independence.
6. Documenting observations, interventions, and evaluations accurately using electronic devices.

Qualifications include:

1. Current license as a Speech Language Pathologist in VA.
2. Certificate of Clinical Competence from the American Speech-Language-Hearing Association (ASHA) or equivalent qualifications, with eligibility to take the ASHA certification exam.
3. Alternatively, relevant education and work experience to qualify for certification, with ongoing supervised work experience.
4. Solid computer skills; experience with electronic medical records preferred.
