Job description

BAYADA Home Health Care is currently seeking an experienced Speech Language Pathologist (SLP) for a part-time or full-time opportunity performing home health visits for our office located in Williston, VT.

This office services adult and geriatric patients in homes throughout Orange and Washington counties.*

Prior home care experience as a Speech Language Pathologist is a plus but not required.

As a home care Speech Language Pathologist, you will be an integral member of a multidisciplinary healthcare team providing rehabilitative care and skilled nursing to clients, enabling them to receive necessary medical care at home.

Speech Language Pathologist, SLP Responsibilities:

1. Make home visits to clients in designated geographic areas.
2. Perform evaluations to assess, diagnose, and treat speech, language, communication, or swallowing difficulties.
3. Develop treatment plans based on evaluations, physician's orders, and goals.
4. Assess and revise care plans, collaborating with BAYADA clinicians as needed.
5. Educate patients and families on rehabilitative care and activities to promote health, safety, and independence.
6. Document observations, interventions, and evaluations accurately using electronic medical records.

Qualifications:

• Current Vermont Speech Language Pathologist license.
• Certificate of clinical competence from the American Speech and Hearing Association or equivalent educational and work experience, with certification process underway.
• Strong computer skills; experience with electronic medical records preferred.
