St. Mary’s Healthcare System for Children in Uniondale, NY, is seeking a Speech-Language Pathologist to provide skilled services per patient care plans for home care patients.

The role focuses on restoring communication, feeding, swallowing, cognitive skills, and overall functional status in a per-visit model. The candidate will evaluate and revise plans of care, document outcomes, and coordinate with the Rehab team and supervisors.

Qualifications

  • Master's degree in Speech-Language Pathology is required.
  • CCC-SLP certification and state licensure are required.
  • Minimum of one year of experience in speech-language pathology and/or pediatric settings.

Responsibilities

  • Provides skilled speech-language pathology services per the plan of care.
  • Implements therapeutic activities to restore function and improve communication, oral-motor skills, feeding and swallowing, cognition, memory and problem solving.
  • Accepts patients referred by Team Supervisor or designee.
  • Demonstrates annual competency in needed skills.
  • Evaluates function and updates plan of care; documents rationale if not addressed.
  • Identifies augmentative communication needs and refers to appropriate vendors.
  • Observes and reports patient response; communicates with Rehab Supervisor and RN/SC as required.
  • Teaches and counsels patient and family on home rehabilitation and exercises.
  • Assesses home safety and coordinates care with team.
  • Coordinates total plan of care and maintains continuity with other health professionals.
  • Participates in patient care conferences to coordinate multi-disciplinary care.
  • Maintains individualized patient progress records and documentation.
  • Submits visit notes weekly and 60-day/discharge/quarterly reports as required.
  • Uses agency educational materials as appropriate.
  • Maintains scheduling flexibility to accommodate department needs.

Description

Responsible for the evaluation, planning, implementation, treatment and documentation of patient status for home care patients. Supports organizations mission to provide the highest quality care by complying with regulatory guidelines, meeting agency standards, and obtaining appropriate certification.

JOB DUTIES / RESPONSIBILITIES
ESSENTIAL DUTIES

1.Provides skilled services in speech pathology in accordance with the authorized practitioner's plan of care,

2.Treats the patient through the use of therapeutic activities designed to restore function and improve communication, oral-motor integrity, feeding and swallowing skills, cognitive status, memory and problem solving activities, hearing and speech fluency and quality and all other SLP functions as stated in their practice act..

3.Accepts only patients referred by the Team Supervisor or designee

4.Demonstrates competency in skills required by the agency on at least an annual basis at the time of field evaluation.

5.Evaluation and assess the patient's level of function by applying diagnostic and prognostic procedures and tests and assists in the development and revision of the plan of care. Assesses the patient's continual care needs. Ensures that the plan of care addresses all problems identified in the assessment or documents rationale for not doing so

6.Identifies specific augmentative communication needs of patient and refers to appropriate vendor

7.Observes, records and reports the patient's reaction to treatment and any changes in the patient's condition to the Team Coordinator or Nurse Manager or Service Coordinator. Reports routinely to Rehab Supervisor and every 30 days to RN or SC. Requests referrals for other disciplines as indicated by the needs of the patient or documents rationale for not doing so.

8.Teaches, supervises and counsels the patient, family and other health care team members regarding home rehabilitation procedures, including the home exercise program, and other care needs as appropriate to the patient's condition

9.Evaluates home safety and coordinates with team members to address issues identified

10.Coordinates the total plan of care and maintains continuity of patient care by liaising with other health professionals assigned to the same patients

11.Attends patient care conferences. Initiates patient care conferences for complex and/or multi-disciplinary patients whenever needed to ensure good coordination of care

12.Develops, prepares and maintains individualized patient care progress records with accuracy, timeliness and according to policies.

13.Submits all visit notes every week or sooner. Submits 60day, D/C summaries and/or quarterly reports as required by the individual program and department

14.Utilizes agency educational material as appropriate.

15.Demonstrates flexibility in scheduling patients as required by department.

NON-ESSENTIAL DUTIES

16.Cooperates with appropriate staff to provide staff education when requested and arranged by the Team Supervisor.

17.Attends meetings and serves on a variety of patient care and strategic planning committees as assigned by the Supervisor or the Director of Rehabilitation.

18.Participates in the agency's quality improvement program.

Requirements
Formal education required:

Master's degree with a major in speech-language pathology.

Holds a certificate of clinical competence and licensure granted by the American Speech and Hearing Association.

Minimum of one year of experience in speech-language pathology and/or pediatric affiliation.
