Job summary

Trilogy Health Services, LLC is seeking a Speech Language Pathologist (SLP) to provide a full range of services including assessment, treatment planning, and interventions in a collaborative setting at Wellbrooke of Carmel, Indiana.

The role requires a Master’s degree in Speech Language Pathology, a current SLP license, and 0-1 years of experience. CCC-SLP from ASHA is preferred or required, with CPR certification also preferred.

Qualifications

• Current state SLP licensure in good standing.
• CCC-SLP from ASHA preferred or required.
• CPR certification preferred.

Responsibilities

• Evaluate patients to identify speech, language, voice, and swallowing disorders.
• Develop and implement individualized therapy plans with measurable goals.
• Provide therapy to improve communication and swallowing abilities.
• Educate patients and families about disorders, therapy techniques, and progress strategies.
• Maintain accurate documentation compliant with facility policies and regulations.
• Collaborate with interdisciplinary team to ensure comprehensive patient care.
