Job summary

Fox Rehabilitation is seeking a licensed Speech Language Pathologist to deliver one-on-one care in Greater Abington, PA and surrounding communities, including private residences and senior living settings.

You'll design evaluation-based plans, provide evidence-based therapy, and document in FOX's EMR while collaborating with PTs, OTs, SLPs, and fitness professionals. Telehealth may be used to extend access and flexibility.

Qualifications

• Must hold a valid Speech Language Pathology license in the state(s) where practicing or be eligible to obtain
• Degree from an accredited Speech Language Pathology program
• Basic computer and EMR proficiency
• Current CPR certification
• Current Certificate of Clinical Competence (ASHA CCC)

Responsibilities

• Provide one-on-one Speech Language Pathology to older adults in homes and community settings
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment
• Deliver therapy sessions that promote wellness, mobility, and independence
• Document effectively in FOX’s EMR in accordance with regulatory standards
• Collaborate with an interdisciplinary team (PTs, OTs, SLPs, Fitness Specialists)
• Educate patients and caregivers to promote safety and aging in place
• Provide telehealth/virtual care when appropriate to extend access

Physical Requirements

FOX clinicians must be able to meet the physical demands of in-home and community-based care, including:

• Exerting medium physical effort, including lifting 25–50lbsoccasionally, 10–25lbsfrequently, and up to 10lbsconstantly

• Performing CPR

• Providing moderate to maximumassistancewith patient transfers and bed mobility

• Repositioning patients, standing/walking for extended periods, climbing stairs, and performing therapeutic activities

• Driving to patient homes and senior living communities as part of the Geriatric House Calls™ model

• Wearing PPE whenrequired
