Job summary

Gallatin Center for Rehabilitation and Healing is seeking a Speech Language Pathologist (SLP) to join our team for full-time and PRN roles. The SLP will assess referred patients, develop treatment plans, and educate patients and families on therapy approaches within a geriatric rehabilitation setting.

The position emphasizes documentation, adherence to HIPAA, and coordination with a multidisciplinary team to provide patient-centered care while maintaining professional standards.

Qualifications

• Graduated from an accredited Speech-Language Pathology program.
• Must hold TN licensure and registration.
• Experience in long-term care or geriatric care preferred.
• Strong documentation skills are essential.
• Experience with evaluations, progress notes, recertifications and discharge documentation.

Responsibilities

• Assess referred patients requiring rehabilitation and provide direct patient care.
• Develop and administer a treatment program with goals based on patient capacity.
• Document patient progress and educate patients and families in therapy methods.
• Collaborate with all disciplines to deliver patient-centered care.
• Ensure compliance with confidentiality, HIPAA, and residents' rights.
