Job summary

Gallatin Center for Rehabilitation and Healing is seeking a Speech Language Pathologist (SLP) to join our team and provide direct patient care within a rehabilitation setting.

The SLP will assess referred patients, establish and administer treatment programs, set goals, document patient progress, and educate families in appropriate therapy methods, while adhering to HIPAA and resident rights.

Qualifications

• Must have graduated from an accredited Speech Therapist college or university.
• Must have a valid professional license and registration for state of TN.
• Experience in long-term care and/or geriatric care is preferred but not required.
• Ability to document thoroughly and maintain patient records.
• Collaborates with all disciplines to provide patient-centered care.

Responsibilities

• Assess referred patients requiring rehabilitation services.
• Develop and supervise a treatment program with goals based on patient capacity.
• Set realistic and achievable goals and document progress.
• Educate patients and families on therapy methods.
• Maintain confidentiality and HIPAA compliance.
