Job summary

Stellar Health Group in Winchester, MA is seeking a licensed Speech-Language Pathologist for a full-time role within our therapy team. You will provide speech therapy services, evaluate patients, develop individualized treatment plans, and collaborate with a multidisciplinary staff to support high-quality rehab care.

Qualifications

• Must hold MA State License as a Speech-Language Pathologist.

Responsibilities

• Provide speech therapy services promoting rehab care quality.
• Evaluate patients promptly as required by state and facility.
• Screen admissions and contribute to treatment planning.

• Deliver treatment per plan and required guidelines.
• Coordinate with other disciplines and staff per treatment plan.
• Maintain routine documentation per Medicare and payor rules.
• Participate in family conferences as requested.
• Submit treatment charges and records timely.

Job Summary

FULL TIME

Responsibilities

• Providing speech therapy services to the facility promoting excellence of rehab care, promoting interdisciplinary cooperation and quality of patients care and promoting the facility geriatric rehab program to the medical and public.
• Evaluate patients within timely fashion, as required by state, payor source or facility requirements
• Screen patients upon admission and/or when requested by director, within a timely fashion
• Develops appropriate plan of treatment and goals for the patient
• Provides treatment as indicated by plan of treatment developed after evaluation
• Communicate with other disciplines and interacts with staff as necessary, as indicated by the treatment plan
• Provide routine documentation according to Medicare, facility, and other payor source guidelines
• Participate in family conferences when requested
• Record and submit in timely fashion all required documentation and treatment charges, according to policy and procedure

Qualifications

• Current MA State License as a Speech-Language Pathologist
• Must have flexibility as a person to be a part of a dynamic and cooperative team, to distribute the workload equitably and according to the individual’s staff position within the department.
• Have the capability to evaluate or assess patients, participate in consultations, teaching and meeting with all levels of residents, staff, family, and other professionals, in order to: plan individualized treatment plans and goals, discuss and plan appropriate discharge plans, and teach and counsel patients and staff
