Qualifications

• Current MA State License as a Speech-Language Pathologist (SLP) in good standing.
• Ability to work within a dynamic, cooperative team and distribute workload equitably.

Responsibilities

• Provide speech therapy services to promote rehab care excellence and interdisciplinary cooperation.
• Evaluate patients promptly as required by state, payor source, or facility requirements.
• Screen admitted patients or upon director request in a timely fashion.
• Develop appropriate treatment plans and goals for patients.
• Provide treatments as indicated by the treatment plan after evaluation.
• Coordinate with other disciplines and staff as needed per the treatment plan.
• Document routinely according to Medicare, facility, and payor guidelines.
• Participate in family conferences when requested.
• Record and submit required documentation and treatment charges timely.

Qualifications for Speech Therapist:

• Current MA State License as a Speech-Language Pathologist
• Must have flexibility as a person to be a part of a dynamic and cooperative team, to distribute the workload equitably and according to the individual's staff position within the department.
• Have the capability to evaluate or assess patients, participate in consultations, teaching and meeting with all levels of residents, staff, family, and other professionals, in order to: plan individualized treatment plans and goals, discuss and plan appropriate discharge plans, and teach and counsel patients and staff

Responsibilities for Speech Therapist:

• Providing speech therapy services to the facility promoting excellence of rehab care, promoting interdisciplinary cooperation and quality of patients care and promoting the facility geriatric rehab program to the medical and public.
• Evaluate patients within timely fashion, as required by state, payor source or facility requirements
• Screen patients upon admission and/or when requested by director, within a timely fashion
• Develops appropriate plan of treatment and goals for the patient
• Provides treatment as indicated by plan of treatment developed after evaluation
• Communicate with other disciplines and interacts with staff as necessary, as indicated by the treatment plan
• Provide routine documentation according to Medicare, facility, and other payor source guidelines
• Participate in family conferences when requested
• Record and submit in timely fashion all required documentation and treatment charges, according to policy and procedure
