Job summary

La Vida Serena Nursing and Rehabilitation in Texas seeks a Full‑Time Speech Language Pathologist to join our in‑house rehab team serving residents through person‑centered care to achieve positive outcomes.

You will evaluate swallowing and cognitive deficits, develop individualized treatment plans, educate residents and caregivers, and coordinate with medical staff to ensure compliant care. New grads welcome to apply; strong team support and competitive benefits offered.

Qualifications

• Active Texas license in Speech-Language Pathology is required.
• In good standing with regulatory agencies and licensing boards.
• SNF/Long term care experience preferred.
• Knowledge of current treatment practices for speech-language pathology.
• Familiarity with Net Health Electronic Documentation System preferred.
• Ability to manage patients with different personalities.

Responsibilities

• Evaluate residents' swallowing or cognitive deficits.
• Assist residents in goals to improve cognition or swallowing.
• Educate residents and caregivers.
• Provide skilled speech and language therapy assessments and develop individualized treatment plans.
• Implement AAC systems for individuals with severe expressive/language disorders.
• Provide training to clients, family, caregivers, and other professionals.
• Coordinate with other professionals for diagnostic swallowing procedures (MBSS, FEES).
• Demonstrate competence in speech therapy standards of practice.
• Communicate changes in residents' condition.
• Prepare accurate billing records and clinical documentation.
• Respond to family requests and concerns promptly.
• Coordinate training with Restorative Nursing and other staff.
• Ensure resident safety and confidentiality; comply with laws and professional standards.

Responsibilities

• Evaluate Residents conditions for swallowing or cognitive deficits.
• Help Residents in their goals to improve Cognition or Swallowing
• Education for Residents and Caregivers
• Provide skilled, medically necessary speech and language therapy assessments as well as develop and implement individualized treatment plans to increase client’s independence and functioning.
• Provide augmentative and alternative communication (AAC) systems for individuals with severe expressive and/or language comprehension disorders.
• Provide training and education to clients, family, caregivers, and other professionals as a subject matter expert.
• Coordinate and collaborate with other professionals for diagnostic swallowing procedures (MBSS, FEES) as ordered by physician.
• Demonstrate competent technical skills according to current Speech Therapy standards of practice.
• Communicate changes in resident’s condition.
• Provide accurate and timely billing records and supporting clinical documentation.
• Respond to family requests and concerns timely.
• Coordinate and provide training with Restorative Nursing and other Care staff.
• Assure resident abuse or neglect does not occur.
• Observe confidentiality and safeguards for all patient information.
• Complies with federal, state, and local legal and professional requirements.

Requirements
