  • Document assessments, treatment sessions, progress notes, and discharge summaries in accordance with facility and regulatory requirements.
  • Collaborate with physicians, nurses, rehabilitation staff, and other interdisciplinary team members to support coordinated patient care.
  • Educate patients, families, and caregivers regarding treatment goals, strategies, and home programs.
  • Participate in quality improvement initiatives, staff meetings, and continuing education activities.
  • Maintain compliance with ethical standards, HIPAA regulations, and state/federal guidelines.

Job Description – Healthcare Facility

Position Summary The Speech‑Language Pathologist (SLP) provides diagnostic and therapeutic services to patients with speech, language, cognitive‑communication, voice, fluency, and swallowing disorders within a healthcare setting. This role ensures the delivery of evidence‑based, patient‑centered care in compliance with facility policies, professional standards, and regulatory guidelines.

Essential Duties and Responsibilities

  • Conduct comprehensive evaluations to assess speech, language, cognitive‑communication, and swallowing function

  • Develop and implement individualized treatment plans based on clinical findings and patient needs

  • Provide direct therapy services to patients across acute, subacute, outpatient, or long‑term care settings

  • Document assessments, treatment sessions, progress notes, and discharge summaries in accordance with facility and regulatory requirements
