Job summary

Fallsview is seeking a Speech Language Pathologist to join their team. The role involves assessing residents, creating individualized care plans, and ensuring accurate documentation and data collection across sections GG and K. You will treat patients, educate families, and collaborate with rehab staff to provide holistic care.

Responsibilities

• Assessment and Evaluation: Conduct thorough assessments of residents' speech, language, cognitive, and swallowing abilities.
• Individualized Care Plans: Develop customized intervention plans to address communication and swallowing difficulties.
• Section GG Data Collection: Ensure accurate and consistent collection of Section GG data for Annual, Quarterly, Falls, and Significant Change assessments, Section K, BIMS, PhQ2-9, and other clinical data as applicable.
• Treatment: Administer speech therapy interventions, including exercises, communication strategies, and swallowing therapy.
• Documentation: Maintain accurate and timely documentation of resident progress and any interventions provided. Comfortable with PoS care delivery and documentation. Maintain up-to-date documentation of resident evaluations, treatment plans, daily and progress notes, HMOs/Managed care progress updates in EMRs.
• Education: Educate residents, family members, and care staff on safe mobility techniques and exercises for continued progress. Conduct remote check-ins through RTM with discharged patients to home or next level of care.
• Interdisciplinary Collaboration: Collaborate with other rehab staff, such as Physical Therapists and Occupational Therapists, to provide holistic care for residents.
