Job summary

Fallsview Rehabilitation & Nursing seeks a dedicated Speech Language Pathologist to join our team. You will assess residents’ speech, language, cognitive, and swallowing abilities and develop individualized intervention plans while collaborating with rehab staff.

Responsibilities include therapy interventions, documentation, and family education, with emphasis on safe, effective communication and swallowing support within a warm, community-focused environment.

Qualifications

• Experience providing speech-language pathology services in a clinical or long-term care setting.

Responsibilities

• Assess residents' speech, language, cognitive, and swallowing abilities.
• Develop individualized intervention plans for communication and swallowing.
• Collect and record data for assessments and progress notes.
• Administer speech therapy treatments and education to residents and families.
• Collaborate with other rehab staff to plan holistic care.
• Document accurately in the EMR and care plans.
