Job summary

YAD Management seeks a Speech Therapist to join Hertford Rehabilitation And Healthcare Center. The role supports residents with comprehensive speech therapy services, from evaluation to discharge, within a collaborative interdisciplinary team.

The candidate should hold an ASHA-recognized MA/MS in Communication Disorders, possess or be eligible for North Carolina licensure, and have CFY completion or near completion, with strong documentation and professional communication skills.

Qualifications

• MA or MS in Communication Disorders recognized by ASHA/state board.
• Current license/registration in the state (as applicable).
• Successful completion of CFY.

Responsibilities

• Conduct comprehensive speech therapy evaluations per MD orders.
• Document evaluation findings clearly in the approved format.
• Create and update comprehensive treatment plans with goals, frequency, duration, and modalities.
• Obtain MD orders for evaluations, treatments, recertifications and discharges.
• Provide therapy using appropriate modalities and adjust as needed.
• Document patient progress in the approved format and timely.
