Job summary

Yanceyville Rehabilitation And Healthcare Center in Yanceyville, NC is hiring a Speech Therapist to evaluate, treat, and document therapy for residents in a long-term care setting. CFY candidates are accepted, with ASHA‑recognized credentials and state licensure required.

Qualifications

• Graduate of an accredited university with an MA, or MS in communication disorders that is recognized by ASHA and state board.
• Holds current license and/or registration in the state (as applicable).
• Successful completion of CFY.

Responsibilities

• Provide comprehensive speech therapy evaluations as per MD orders.
• Document findings on the standardized evaluation format neatly, accurately and adhering to guidelines.
• Provide a comprehensive treatment plan including long and short term goals, frequency, duration and treatment modalities.
• Ensure MD orders are obtained for evaluations, treatments, recertifications and discharges.
• Provide comprehensive treatment to patients utilizing modalities and modifying patient treatment as indicated and adhering to precautions.
• Document patient’s progress in approved format accurately and timely.
• Document monthly updated treatment plan in standardized format in timely manner.
• Document patient’s discharge and in-service as indicated.
• Document any caregiver or discharge in-services in approved format as indicated.
• Complete any and all other documentation required by Facility.
• Document care plans and information needed for the MDS as deemed necessary by the facility.
• Attend facility and rehab meetings as indicated by supervisor and/or Regional Manager.
• Participate in facility committee meetings as indicated.
• Act as a rehab delegate to the interdisciplinary team.
• Participates in scheduling of patients.
• Perform screens as indicated.
• Complete billing timely and bill patients ethically and accurately for all speech therapy services rendered.
• Cover other therapists’ patient caseload during absence as necessary.
• Supervise junior staff members or students in compliance with State ST Board and Federal regulations and requirements.
