Job summary

Twinoakscare is seeking a Full-Time Speech Language Pathologist to join our in-house rehab team serving residents with person-centered care to achieve positive outcomes. You will conduct assessments, develop and implement treatment plans, and provide AAC systems as needed, while collaborating with care staff and families.

Qualifications

• Active/Valid Texas license in Speech-Language Pathology.
• SNF/Long term care experience preferred.
• Knowledge of Net Health Electronic Documentation System preferred.

Responsibilities

• Evaluate residents’ swallowing or cognitive deficits.
• Develop and implement individualized treatment plans to improve independence and functioning.
• Provide skilled speech and language therapy assessments and AAC systems.
• Educate residents, caregivers, and other professionals.
• Coordinate with other professionals for diagnostic swallowing procedures as ordered by physician.
• Maintain accurate billing records and clinical documentation.
• Ensure confidentiality and compliance with all regulations.
