Job summary

Qualifications

Responsibilities

• Evaluate a wide variety of patient types and identify dysfunction to guide treatment.
• Perform initial evaluations and establish treatment plans with measurable goals.
• Collaborate with interdisciplinary services to refer patients as needed.
• Document progress notes, discharge summaries and patient progress reports.
