Crestwood Health & Rehabilitation in Wills Point, TX, is seeking a PRN Speech-Language Pathologist to join our in-house therapy team. You will screen and treat patients with communication, cognitive, and swallowing disorders, developing care plans and discharge summaries in compliance with regulatory standards.

Qualifications

• Licensed Speech-Language Pathologist in Texas.
• PRN position; open to all experience levels.
• Ability to screen, evaluate and plan treatment for speech, cognitive and swallowing disorders.

Responsibilities

• Screen and evaluate patients with communication, cognitive, or swallowing disorders and create care plans.
• Deliver rehabilitative treatment for speech, cognitive, and swallowing disorders with regulatory compliance.
• Evaluate outcomes and modify services; refer as needed.
• Provide counseling to patients, families, and other providers about speech disorders.
• Generate comprehensive discharge summaries per regulatory and clinical requirements.

SPEECH-LANGUAGE PATHOLOGIST - PRN
CRESTWOOD HEALTH & REHABILITATION - WILLS POINT, TX

Crestwood Health & Rehabilitation in Wills Point, TX, is a post-acute care facility with long-term care, short-term care, and geriatric outpatient services provided. We are seeking a PRN Speech-Language Pathologist to join our in-house therapy team. Key Responsibilities:

• Screen and evaluate patients with communication, cognitive, or swallowing disorders, developing appropriate care plans following regulatory and clinical standards.
• Deliver rehabilitative treatment to patients with communication, cognitive, and/or swallowing disorders, ensuring compliance with regulatory and clinical practice requirements.
• Evaluate treatment outcomes, modifying services based on patient evaluation and making referrals as needed.
• Provide consultation and counseling to patients, families, caregivers, and other service providers related to speech disorders.
• Generate comprehensive discharge summaries in adherence to regulatory and clinical requirements.

Qualifications:

Speech-Language Pathologist license is required.
