Job summary

Phoenix Home Care and Hospice is seeking a part-time Speech-Language Pathologist to provide patient-centered services in the Columbia, MO area. You will assess, deliver, supervise, and evaluate therapy per physician Plan of Care, ensuring compliance with regulations and agency policies.

Responsibilities include direct therapy, documentation, and collaborative care with the team. Travel may be required; part-time benefits are offered to support work-life balance.

Qualifications

  • Master’s degree in Speech-language pathology and active state license.
  • Prefer 1 year of clinical experience in acute care or rehabilitation.
  • Ability to work in a fast-paced, sometimes stressful environment with good judgment.
  • Mental alertness, precision, analytical ability, problem solving and initiative.
  • Travel may be required occasionally.

Responsibilities

  • Performs direct speech therapy services to patients as ordered by a physician and per Plan of Care.
  • Complies with agency standards, policies, procedures, and payer requirements.
  • Provides legible, accurate, timely documentation of care provided.
  • Communicates with team to review and update the Plan of Care.
  • Works toward agency productivity goals; performs other duties as assigned.

Speech-language therapy plays an essential role in the home health model of care. It provides valuable support to patients with various speech, language, communication, cognitive, and swallowing disorders. Whether individuals face difficulty swallowing, challenges with memory and disorientation, or an inability to effectively communicate, speech-language therapy in the home offers significant benefits.

Responsibilities
  • Performs direct speech therapy services to patients as ordered by a physician and in accordance with the Plan of Care.
  • Complies with agency standards, policies, procedures, state and federal regulations, state practice acts for licensed personnel, the Business Code of Ethics, and payer requirements.
  • Provides legible, accurate, timely and complete documentation in accordance with the established agency standards of all care and services provided to patients as assigned.
  • Communicates and collaborates with team members when reviewing, revising, and updating the patient’s Plan of Care.
  • Performs job responsibilities in accordance with agency financial and productivity goals in the promotion of effective and efficient agency operations.
  • Performs any other duties as assigned by supervisor.
Requirements
  • Education / Licensure: Master’s degree in Speech-language pathology. Current license in the state(s) of practice.
  • Experience: Prefer one (1) year clinical experience, preferably in an acute care or rehabilitation setting.
  • Ability to work in a fast-paced, stressful environment, utilizing good judgment, flexibility and patience in all situations.
  • Position requires mental alertness, precision, analytical ability, problem solving, memory, communication, concentration and initiative.
  • May require frequent travel and irregular hours.
