Job summary

Ingleside Communities is seeking a part-time Speech-Language Pathologist to evaluate, diagnose, and treat individuals with speech, language, cognitive, voice, and swallowing disorders. You will develop personalized treatment plans, provide therapy, document progress, and collaborate with physicians, nursing staff, and families to support patients’ quality of life.

You will also educate caregivers, participate in care planning, and ensure safe swallowing and continuity of care, with opportunities

Qualifications

  • Master’s or doctoral degree in Speech Language Pathology and state license.
  • Completion of 350 clock hours supervised practicum (or in progress) and 9 months of supervised full-time SLP experience with national exam.
  • Currently licensed in the state(s) where services are furnished.

Responsibilities

  • Improves or maximizes the communication of the patient.
  • Returns the individual to optimum and productive living within the patient’s capabilities.
  • Participates with home care personnel in patient care planning.
  • Provides full range Speech Language Pathology Services as ordered by physician.
  • Directs and supervises personnel as assigned.
  • Performs all skilled procedures.
  • Writes reports to physicians regarding patient’s progress.
  • Instructs patients and families in home programs.

Education

Master’s or doctoral degree in Speech Language Pathology
Licensed as a Speech Language Pathologist in state of practice
350 clock hours of supervised clinical practicum
9 months of supervised full-time SLP experience
National examination approved by the Secretary
Currently licensed in the practicing state(s)

Job Summary

The Speech-Language Pathologist evaluates, diagnoses, and treats individuals with speech, language, communication, cognitive, voice, and swallowing disorders. This position develops individualized treatment plans, provides therapy, documents progress, and collaborates with physicians, nursing staff, rehabilitation professionals, families, and caregivers to support each patient’s functional abilities and quality of life. The Speech-Language Pathologist also provides education and recommendations to promote safe swallowing, effective communication, and continuity of care.

Key Responsibilities
  • Improves or maximizes the communication of the patient.
  • Returns the individual to optimum and productive living within the patient’s capabilities.
  • Periodically participates with all other home care personnel in patient care planning.
  • Provides full range Speech Language Pathology Services as ordered by physician.
  • Directs and supervises personnel, as assigned.
  • Takes initial history and makes initial evaluation.
  • Performs all skilled procedures.
  • Consults with physicians regarding change of treatment.
  • Writes reports to physicians regarding patient’s progress.
  • Instructs patients and family members in home programs.
  • Periodically presents an in-service to staff
  • An initial evaluation, including plan of treatment and goals, must be completed and submitted to the physician for approval. A recertification by physician every 60 days is necessary if further treatment is to be continued. A progress note is written each visit. A reevaluation is written when expected duration of treatment is reached. A summary is written upon patient’s discharge.
  • Assists the physician in evaluating level of function.
  • Helps develop the plan of care and revises as necessary.
  • Prepares clinical and progress notes.
  • Advises and consults with the family and other Agency personnel.
  • Participates in in-service programs.
  • Completes and submits OASIS assessments, reassessments, transfers, resumptions of care, discharges and significant change in condition in accordance with Agency defined time frames.
  • Appropriately utilizes ICD-10 codes.
  • Participates in QAPI activities as assigned.
Qualifications

Required:

  • A person who has a master’s or doctoral degree in Speech Language Pathology, and is licensed as a Speech Language Pathologist by the state where they furnish services, or
  • A person who has successfully completed 350 clock hours of supervised clinical practicum (or be in the process of completing), at least nine months of supervised full‑time SLP experience, and has successfully completed a national examination approved by the Secretary.
  • Currently licensed in the state(s) in which practicing.

Preferred:

Long-term care or skilled nursing experience

  • Two (2) years’ experience, preferred but not required
