Job summary

CHI St. Vincent Health at Home is seeking a skilled Speech-Language Pathologist to assess, diagnose, and treat speech, language, and swallowing disorders in patients’ homes. You will develop individualized treatment plans and collaborate with families and healthcare professionals to achieve optimal outcomes.

The role involves providing therapy at patients' homes and ensuring accurate documentation, with emphasis on family training and support for home-based rehabilitation.

Qualifications

• Masters degree in Speech-Language Pathology from an accredited program.
• Active registered SLP license in the state you practice and CCC-SLP certification from ASHA.
• Must obtain and maintain CPR/BLS Certification.
• One-year Speech Therapy experience in a clinical setting.
• Current Driver's License.
• Compliance with federal, state and local laws and regulations.

Responsibilities

• Provide therapy to patients in their homes, including techniques to improve speech, language, cognitive-communication, and swallowing skills.
• Customize therapy strategies to meet individual patient goals and objectives.
• Maintain accurate and timely documentation of patient progress, treatment goals, and outcomes in accordance with regulatory requirements and standards.

The Speech Therapist will assess, diagnose and treat patients with speech, language, communication and swallowing disorders for patients in their homes.
