Job summary

Lock Haven Rehabilitation & Senior Center in Lock Haven, PA is seeking a dedicated and compassionate PRN Speech-Language Pathologist (SLP/CCC) to join our in-house rehabilitation team. The role focuses on residents in a Skilled Nursing Facility / Long-Term Care setting, with emphasis on communication, cognition, and swallowing goals.

Responsibilities

• Evaluate residents with speech, language, cognitive, voice, and swallowing disorders.
• Develop and implement individualized treatment plans.
• Provide skilled therapy services to improve communication, cognition, and swallowing function.
• Educate residents, families, and caregivers regarding goals and progress.
• Complete timely and accurate documentation.
• Collaborate with physicians, nursing staff, and rehabilitation team members.
• Participate in interdisciplinary care planning and discharge recommendations.

Lock Haven Rehabilitation & Senior Center | Lock Haven, PA

Lock Haven Rehabilitation & Senior Center is seeking a dedicated and compassionate PRN Speech-Language Pathologist (SLP/CCC) to join our in-house rehabilitation team. This position offers the opportunity to work with a collaborative interdisciplinary team in a Skilled Nursing and Long-Term Care setting focused on helping residents achieve their highest level of communication, cognition, and swallowing function.

Qualifications
