Job summary

Phoenix Home Care & Hospice is seeking a part-time Speech-Language Pathologist to provide home-based therapy in Lebanon, Missouri and surrounding areas. You will deliver high-quality speech, language, cognitive and swallowing therapy to patients in their homes, aligning plans with physician orders and the agency plan of care.

Qualifications

• Master’s degree in Speech-language pathology and current licensure.
• On-site availability in Lebanon, MO and surrounding areas is required.
• Prefer one year of clinical experience in acute care or rehabilitation settings.
• Able to work in a fast-paced environment with good judgment, flexibility and patience.
• Must be mentally alert with strong analytical, memory and communication skills.

Responsibilities

• Performs direct speech therapy services to patients as ordered by a physician.
• Documents care accurately and timely per agency standards.
• Collaborates with the care team to review and update patient Plans of Care.
• Supports efficient agency operations and adheres to ethics and payer requirements.
• Performs additional duties as assigned by supervisor.

Responsibilities

• Performs direct speech therapy services to patients as ordered by a physician and in accordance with the Plan of Care.
• Complies with agency standards, policies, procedures, state and federal regulations, state practice acts for licensed personnel, the Business Code of Ethics, and payer requirements.
• Provides legible, accurate, timely and complete documentation in accordance with the established agency standards of all care and services provided to patients as assigned.
• Communicates and collaborates with team members when reviewing, revising, and updating the patient’s Plan of Care.
• Performs job responsibilities in accordance with agency financial and productivity goals in the promotion of effective and efficient agency operations.
• Performs any other duties as assigned by supervisor.

Requirements

• Education / Licensure: Master’s degree in Speech-language pathology. Current license in the state(s) of practice.
• Experience: Prefer one (1) year clinical experience, preferably in an acute care or rehabilitation setting.
• Ability to work in a fast-paced, stressful environment, utilizing good judgment, flexibility and patience in all situations.
• Position requires mental alertness, precision, analytical ability, problem solving, memory, communication, concentration and initiative.
• May require frequent travel and irregular hours.
