Job summary

Phoenix Home Care and Hospice is seeking a part-time Speech-Language Pathologist to provide direct therapy to home health patients in Lebanon, MO and surrounding areas under physician orders and the Plan of Care.

You will document care per agency standards, collaborate with the care team, and adapt to a flexible schedule with travel as needed. The role emphasizes work-life balance and supportive home health teamwork.

Qualifications

• Master's degree in Speech-language pathology and active state licensure.
• 1 year clinical experience preferred.
• Ability to work in a fast-paced environment and maintain composure.
• May require frequent travel and irregular hours.

Responsibilities

• Provide direct speech therapy services to patients as ordered by a physician and per the Plan of Care.
• Comply with agency standards, policies, procedures, and payer requirements.
• Maintain legible, accurate, timely documentation of care provided.
• Collaborate with the team when reviewing and updating the Plan of Care.
• Align job responsibilities with agency financial and productivity goals.
• Perform other duties as assigned by supervisor.

Responsibilities

• Performs direct speech therapy services to patients as ordered by a physician and in accordance with the Plan of Care
• Complies with agency standards, policies, procedures, state and federal regulations, state practice acts for licensed personnel, the Business Code of Ethics, and payer requirements
• Provides legible, accurate, timely and complete documentation in accordance with the established agency standards of all care and services provided to patients as assigned
• Communicates and collaborates with team members when reviewing, revising, and updating the patient's Plan of Care
• Performs job responsibilities in accordance with agency financial and productivity goals in the promotion of effective and efficient agency operations
• Performs any other duties as assigned by supervisor

Requirements

• Education / Licensure: Master's degree in Speech-language pathology. Current license in the state(s) of practice
• Experience: Prefer one (1) year clinical experience, preferably in an acute care or rehabilitation setting
• Ability to work in a fast-paced, stressful environment, utilizing good judgment, flexibility and patience in all situations
• Position requires mental alertness, precision, analytical ability, problem solving, memory, communication, concentration and initiative
• May require frequent travel and irregular hours
