Legend Oaks Health & Rehabilitation in Gladewater, TX is seeking a part-time Speech-Language Pathologist to join our in-house therapy team, serving geriatric patients in post-acute and long-term care settings. The ideal candidate holds an SLP license and is open to all experience levels.

You will screen, treat, assess outcomes, and provide counseling to patients and families while delivering compliant, high-quality care.

Qualifications

• SLP license required to practice.
• Open to SLPs at all experience levels.

Responsibilities

• Screen and evaluate patients with communication, cognitive, or swallowing disorders and develop care plans.
• Deliver rehabilitative treatment for communication, cognitive, and swallowing disorders.
• Evaluate outcomes and modify services; make referrals as needed.
• Provide counseling to patients, families, caregivers, and other providers.
• Generate discharge summaries in line with regulatory requirements.

Legend Oaks Health & Rehabilitation in Gladewater, TX, is a post-acute care facility with long-term care, short-term care, and geriatric services provided. We can offer a stable and fun in-house therapy team to work with as well as excellent programs serving our geriatric clientele in their journey to recovery. We are seeking a Part Time Speech-Language Pathologist to join our in-house therapy team.

Key Responsibilities:

• Screen and evaluate patients with communication, cognitive, or swallowing disorders, developing appropriate care plans following regulatory and clinical standards.
• Deliver rehabilitative treatment to patients with communication, cognitive, and/or swallowing disorders, ensuring compliance with regulatory and clinical practice requirements.
• Evaluate treatment outcomes, modifying services based on patient evaluation and making referrals as needed.
• Provide consultation and counseling to patients, families, caregivers, and other service providers related to speech disorders.
• Generate comprehensive discharge summaries in adherence to regulatory and clinical requirements.

Qualifications:

Speech-Language Pathologist license is required.
This position is open to SLPs at all experience levels.
