Speech-Language Pathologist (SLP) – Home Health

Location: Corvallis, OR (Travel Required within a 60-mile radius)

Position Type: Full-Time, Traveling

Job Overview:
We are seeking a dedicated and compassionate Speech-Language Pathologist (SLP) to join our growing home health team. In this role, you will provide essential speech therapy services to patients in their homes, helping them improve communication, voice, and swallowing functions. You will work closely with an interdisciplinary team and offer personalized care tailored to each patient’s needs.

Key Responsibilities:

  • Perform comprehensive speech and language assessments in the home environment to set therapy goals.

  • Develop and execute individualized therapy plans for patients with speech, language, and swallowing disorders.

  • Implement therapy using specialized techniques and equipment tailored to the patient's specific condition.

  • Recommend communication aids or devices as necessary.

  • Collaborate with referring physicians and the clinical team to coordinate care.

  • Participate in case conferences and interdisciplinary meetings to ensure effective care delivery.

  • Accurately document evaluations, treatment plans, and patient progress.

  • Stay current with advancements in speech-language pathology practices.

  • Provide education and support to patients, caregivers, and family members regarding therapy goals and methods.

  • Supervise home health aides involved in speech-language care when applicable.

Qualifications:

  • Master’s degree in Speech-Language Pathology from an accredited program.

  • Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) from ASHA.

  • Valid state license to practice as an SLP in Oregon.

  • Minimum of one year of clinical experience in speech-language pathology (home health experience preferred).

  • Current BLS (CPR) certification.

  • Must have a valid driver’s license, reliable transportation, and active auto insurance that meets state minimum requirements.

Work Requirements:

  • This is a traveling role, covering a 60-mile radius from the base location in Salem, OR.

  • Must be comfortable with frequent driving.
