Job summary

Alternate Solutions Health Network is seeking a Speech Language Pathologist for its home health program serving Canton, OH and surrounding territories. The role delivers skilled therapy as ordered by physicians and ensures patient safety and high-quality documentation.

You will educate patients and families, collaborate with a multidisciplinary team, and participate in care planning and transitions while maintaining compliance with Medicare and state guidelines.

Qualifications

  • Current Speech-Language Pathologist (SLP) license in the state of employment.
  • Minimum of 1 year experience as an SLP preferred.
  • Home care experience is desired.
  • Ability to communicate effectively with patients, families, physicians and coworkers.
  • Knowledgeable on speech therapy best practices.

Responsibilities

  • Provides skilled speech therapy as ordered by the attending physician.
  • Evaluates the patient’s level of functioning and develops an individualized plan of care.
  • Documents all treatment, assessments and patient education per agency and Medicare/Medicaid guidelines.
  • Educates patient and family on disease, therapy plan and home exercise programs.
  • Collates patient progress and coordinates with scheduling and admissions.

The SLP educates patients and their family members and ensures the safety of the patient. The SLP is accountable for completing accurate documentation and remains compliant with all legal rules and regulations. The Company adopts the speech-language pathologist scope of practice as outlined in Chapter 4753 ORC for its licensed SLP personnel. Ability to make appropriate speech therapy judgments.Ability to identify a situation and handle it with the best possible solution.Detail-oriented and observant.Disciplined style of work ethic with the ability to prioritize and be timely.Valid driver’s license and auto insurance with your name as a listed driver.MAJOR AREAS OF RESPONSIBILITY:Provides skilled speech therapy as ordered by the attending physicianEvaluates the patient’s level of functioning by applying diagnostic and prognostic proceduresDevelops an individualized ST plan of care and makes adjustments as needs changeObserves, records, and reports patient reactions, changes, and progress to attending physician and any appropriate agency staffInforms health care staff through certain phases of speech therapy which they may use to work with the patientUses activities and home exercise programs to improve swallowing, breathing, cognition, speech and communication, and/or any other speech therapy related functionGuides the patient in his/her use of therapeutic, creative and self-care activities for the purpose of improving functioningPromotes continuity of care with appropriate admissions, transfers and dischargesCounsels and instructs patient and family on the disease/injury/disability and how to manage in all phases of speech therapyIncorporates patient and family in development of plan of careInstructs patient and family members on proper use of procedures and any equipmentHelps decrease re-hospitalizations by teaching the patient on preventative measures and making good decisionsListens to patient and family membersCompletes all clinical documentation in accordance with agency protocol and Medicare/Federal guidelinesDocuments all aspects of treatment, assessments, and patient educationCompletes chart audits at agency location as requestedCommunicates with scheduler any changes as neededParticipates in all on-call requirements and case conferencesAttends in-service trainings and mandatory agency meetingsParticipates in education programs and in-services as mandated by state lawParticipates in orientation/training of new employeesMaintains a professional appearance as a representative of the companyUnderstands and practices agency policies and proceduresHEALTH QUALIFICATIONS:Health Requirements: Must be able to meet all physical position requirements. Evidence required of a current negative tuberculin skin test or x-ray or receive TB testing at the date of orientation. Thereafter an annual tuberculin skin test is required, unless the employee has documentation of a previous TB test. Employee must be free of physical/medical conditions, which would limit or restrict their ability to perform the job functions listed below.
