Job summary

Angels Care Home Health is seeking a Speech Language Pathologist to deliver in-home therapy for patients with speech, language, swallowing, and cognitive disorders in Anna, TX.

The role involves planning, implementing, and evaluating patient care plans, and collaborating with an interdisciplinary team to ensure optimal outcomes. A Master’s in Speech Language Pathology, state licensure, CCC-SLP, and a valid driver’s license are required, with 2 years in acute care or rehab preferred.

Qualifications

• Master’s degree in Speech Language Pathology from an accredited college or university.
• Current state license in Speech Language Pathology.
• Certificate of Clinical Competence from American Speech Language Hearing Association (ASHA).
• Valid driver’s license.
• 2 years’ experience in an acute care or rehabilitation setting; home health preferred.
• Reliable transportation with current auto liability insurance.

Responsibilities

• Provides coordinated care to patients in their place of residence.
• Plans, implements, and evaluates patient care plans to restore or maintain patient well-being.
• Collaborates with interdisciplinary team.
