Job summary

FOX Rehabilitation is expanding in Ohio and seeks licensed Speech Language Pathologists to deliver one-on-one geriatric care in patients' homes or senior living communities. Clinicians enjoy autonomy with flexible schedules and the option to incorporate telehealth into their practice.

Responsibilities include comprehensive evaluations, individualized plans of care, and evidence-based therapy, with collaboration across an interdisciplinary team to maximize patient independence and quality of life.

Qualifications

• Valid Speech Language Pathology license in the state(s) of practice or eligibility to obtain.

Responsibilities

• Provide one-on-one Speech Language Pathology care to older adults in home or senior living settings.
• Perform evaluations, develop plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions that support wellness, longevity, mobility, and independence.
• Document effectively within FOX’s EMR system according to regulatory standards.
• Collaborate with an interdisciplinary team (PTs, OTs, SLPs, Fitness Specialists).
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Provide telehealth/virtual care when appropriate.

Qualifications

• ValidSpeech Language Pathologylicense in the state(s) of practice, or eligibility to obtain
• Degree from an accreditedSpeech Language Pathologyprogram
• Basic computer and EMRproficiency
• Current CPR certification
• Current Certificate of Clinical Competence (ASHA CCC)

Physical Requirements

FOX clinicians must be able to meet the physical demands of in-home and community-based care, including:

• Exerting medium physical effort, including lifting 25-50lbs occasionally, 10-25lbs frequently, and up to 10lbs constantly
• Performing CPR
• Providing moderate to maximum assistance with patient transfers and bed mobility
• Repositioning patients, standing/walking for extended periods, climbing stairs, and performing therapeutic activities
• Driving to patient homes and senior living communities as part of the Geriatric House Calls model
• Wearing PPE when required

In Ohio, the standard base pay range for a Full-Time role is $60,000 - $95,000 annually with an opportunity to earn more. Part-Time and FOX Flex (PRN) will be pro-rated.
