Job summary

FOX Rehabilitation is seeking a licensed Speech Language Pathologist to deliver one-on-one therapy in private homes and senior living communities in New York. You will assess, plan, and implement evidence-based treatment while maintaining thorough documentation and an autonomous, flexible schedule.

Qualifications

• Valid Speech Language Pathology license in New York or eligibility to obtain.
• Degree from an accredited Speech Language Pathology program.
• Basic computer and EMR proficiency.
• Current CPR certification.
• Current Certificate of Clinical Competence (ASHA CCC).

Responsibilities

• Provide one-on-one Speech Language Pathology to older adults in homes or senior living settings.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions that support wellness, longevity, mobility, and independence.
• Document effectively within FOX’s EMR system per regulatory standards.
• Collaborate with an interdisciplinary team (PTs, OTs, SLPs, Fitness Specialists).
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Provide telehealth/virtual care when appropriate to extend access.

Qualifications

Physical Requirements

• Exerting medium physical effort, including lifting 25–50 lbs occasionally, 10–25 lbs frequently, and up to 10 lbs constantly
• Performing CPR
• Providing moderate to maximum assistance with patient transfers and bed mobility
• Repositioning patients, standing/walking for extended periods, climbing stairs, and performing therapeutic activities
• Driving to patient homes and senior living communities as part of the Geriatric House Calls™ model
• Wearing PPE when required
