Job summary

Mosaic Health, LLC. is seeking a Speech-Language Pathologist (PRN) for our Home Health Department in Naples, FL. You will assess patients in their homes and deliver evidence-based therapy to improve communication and swallowing safety, while promoting overall quality of life.

You will work autonomously within a supportive team, collaborating with other health professionals and documenting progress, with strong emphasis on patient-centered care and HIPAA compliance.

Qualifications

• Current and valid Speech-Language Pathologist license in Florida.
• Minimum of 1 year of experience in hospital, rehab, or skilled nursing facility is required.
• Strong clinical skills, excellent communication, and a patient-centered approach.

Responsibilities

• Conduct comprehensive evaluations of patients' speech, language, cognitive-communication, and swallowing abilities.
• Develop individualized treatment plans targeting specific goals (speech sounds, language, fluency, voice, swallowing).
• Provide direct therapy services (articulation, language, fluency, voice, swallowing) using evidence-based practices.
• Educate patients and families on strategies to improve communication and swallowing safety.
• Collaborate with physicians, occupational therapists, and physical therapists to create care plans.
• Maintain accurate documentation of evaluations, treatment plans, progress notes, and discharge summaries.
• Adhere to HIPAA and Medicare regulations.

How will you make an impact & Requirements

What You'll Do

• Patient Assessment: Conduct comprehensive evaluations of patients' speech, language, cognitive-communication, and swallowing abilities to identify strengths, weaknesses, and treatment needs.
• Treatment Planning: Develop individualized treatment plans that address specific patient goals, including speech sound production, language development, fluency, voice disorders, and swallowing difficulties.
• Therapeutic Interventions: Provide direct therapy services, such as articulation therapy, language therapy, fluency therapy, voice therapy, and swallowing therapy, using evidence-based practices.
• Patient Education: Educate patients and their families on strategies to improve communication skills, swallowing safety, and overall quality of life.
• Collaboration: Collaborate with other healthcare professionals, such as physicians, occupational therapists, and physical therapists, to develop comprehensive care plans and ensure optimal patient outcomes.
• Documentation: Maintain accurate and timely documentation of patient evaluations, treatment plans, progress notes, and discharge summaries.
• Compliance: Adhere to all relevant regulatory standards and guidelines, including HIPAA and Medicare regulations.

What We're Looking For

• Licensure: Current and valid Speech-Language Pathologist (SLP) license in the state of Florida.

Compensation Range

Compensation Range: $81,492.00 to $122,238.00 The anticipated base salary range represents the Company's good-faith estimate of the compensation it reasonably expects to pay for this position at the time of posting. Actual compensation will be determined based on factors including experience, skills, qualifications, geographic location, internal equity, and business needs.
